Telehealth consultation & compounding consent.

Your evaluation is conducted remotely using the information you provide in the clinical questionnaire, together with any follow-up messages or calls. No in-person physical examination is performed.

Telehealth has limits. A remote physician cannot palpate, auscultate or order same-day imaging, and technology or incomplete information can affect a clinical judgment. You agree to answer accurately and completely, and to tell the clinical team about every medication and condition, including supplements.

Telehealth is not for emergencies. If you have chest pain, difficulty breathing, an allergic reaction, thoughts of self-harm or any other urgent symptom, call 911 or go to the nearest emergency department.

A prescription is not guaranteed. The physician reviewing your chart may decline treatment, change the requested medication, alter the dose or require labs first. Declining is a clinical decision made in your interest, and your payment is refunded in full with no consultation fee retained.

You may withdraw from care at any time, and you may request that your chart be transferred to another clinician.

Your protocol is prepared by a US licensed compounding pharmacy: a 503A pharmacy dispensing patient-specific prescriptions, or a 503B outsourcing facility registered with the FDA and inspected against current good manufacturing practice (cGMP) requirements.

Compounded medications are prepared for an individual patient and are not FDA-approved products. The FDA does not review compounded formulations for safety, effectiveness or manufacturing quality in the way it reviews brand-name drugs.

Batches are tested for potency, sterility (USP <71>) and endotoxins where the dosage form requires it, and every unit is lot-traceable to its certificate of analysis.

Every medication carries risk, including side effects, allergic reaction and interaction with other drugs. Your physician discusses the risks specific to your protocol and provides written instructions with your shipment.

Alternatives always exist, including brand-name FDA-approved products, in-person care, lifestyle-only approaches and no treatment at all. You are free to choose any of them.

By submitting the clinical intake you confirm you are at least 18 years old, physically located in a state where our physicians are licensed, and that you have read and understood these disclosures and consent to remote evaluation and, if approved, treatment with a compounded medication.
